At least 13 people have died and about 50 are injured after two passenger train collided in India’s Southern Andhra Pradesh State on Sunday.

A rescue operation was launched and hundreds of emergency workers were at the site to clear the wreckage.

Officials said a preliminary investigation has found that a “human error” had led to the collision.

Prime Minister Narendra Modi expressed his condolences and said he was in touch with the railways minister.

The crash took place in the Vizianagaram district on Sunday evening.

Officials said three carriages of a passenger train, travelling between Visakhapatnam and Palasa, derailed around 13:30 GMT, after it was hit by another train.

Sunday’s accident comes just months after a devastating crash involving three trains killed 292 people and injured thousands more in the Eastern State of Odisha.

The crash happened in Andhra Pradesh state’s Vizianagaram district when an incoming train slammed into a stationary train, leading to derailment of at least three rail cars, senior railway officer Saurab Prasad said.

Prasad said a preliminary investigation found that human error caused the accident.

Train crashes are common in India and are often blamed on human error or outdated signaling equipment.

In June, more than 280 people were killed in one of the country’s deadliest rail crashes in decades when two passenger trains rammed into each other in eastern India.

More than 12 million people ride 14,000 trains across India every day, traveling on 64,000 kilometers (40,000 miles) of track.
